Dates: Monday 7 June to Friday 11 June 2021
Time: 9:00am and 3:00 pm on each day
You will be expected to do the following to access the test:
1. An “Individual and Vulnerability Assessment Form” on https://www.com.ui.edu.ng/covid-assessment/covid-assessment.php (for COMUI students only). An assessment score will be generated and sent to your email. Kindly download the documents and code. Please come to the testing venue with your code.
2. The “NCDC electronic case investigation form” on ecif.ncdc.gov.ng
There are two options on the page; “Rapid Test” and “Laboratory Test”. Kindly select the “Rapid Test” option, complete the form and come to the venue with the code generated as well.
Please come to the testing venue with the TWO DIFFERENT CODES.
