Biography:

I am a psychiatrist with sub-specialty interest in older people’s mental health and wellbeing. My current research is focused on developing an evidence base on which to support policy for improvements in the mental health and wellbeing of older people in Nigeria and across sub-Saharan Africa (SSA). Specifically, I have used the tools of epidemiology and evidence-based medicine to highlight burden, identify risk factors, outcomes, intervention nodes as well as the most culturally appropriate, resource-aligned interventions for mental health conditions among older people in the general Nigerian population as well as in subpopulations with stroke and Parkinson’s disease (PD). I have been part of multidisciplinary teams conducting prospective epidemiological studies on dementia and mental health of older adults living in communities spread across south-Western and north-Central Nigeria. In these studies, we have shown that the lower incidence of dementia in Nigeria appears unrelated to higher dementia mortality, but that a lower gait speed and diminished cognitive reserve may be important early markers of dementia in this population.

In addition, we found that contrary to implicit information from the W.H.O Global Burden of Diseases, chronic pain, especially when non-localised, demonstrates a stronger association with directly measured disability compared with several other chronic health disorders in older people from Nigeria and many low/middle income countries. Also, and contrary to the notion of ‘African somatization of depression’, we found that there are broad similarities in psychological and somatic symptom expressions of depression between older Nigerians and their European counterparts. I have in the course of my work proposed notable models for the ‘African collective unconscious’, and ‘compression of disability’ until later-life in Nigerians.

Due to an observed scarcity of information on the risk factors and the expression of behavioural and psychological symptoms (BPSD) in older Africans with stroke and PD, I have led studies seeking answers to the question about how BPSD impact on disease processes and outcomes? Also included in these studies is the wellbeing of caregivers. We have found that BPSD may precede motor and cognitive symptoms of stroke and PD. Compared with motor symptoms, BPSD were also more distressing to patients and their caregivers and are the main reasons for cognitive impairment and deteriorating physical functioning in the context of disease.

In collaboration with other colleagues, I developed and validated the first stroke specific screening tool for depression (‘HRQOLISP-E’) as well as the short neurological examination scale (S-NES).  Also, as part of my work with other colleagues, we identified the vulnerability-marking neurological soft sign (NSS) in indigenous Africans with schizophrenia. We also found cheap and easy to measure NSS that are important proxies of expensive single neucleotide and microsatellite-based technologies for biogeographical ancestry determination in genomic research. I have also worked in collaboration with colleagues to improve the quality of mental healthcare available to older persons living in Nigeria and SSA. In these works, I have critically examined global best practices and how well they might fit into the uniquely challenging healthcare environment in SSA. We found that a combination of depot antipsychotic with an assertive monitoring program to promote medication adherence was an effective and safe intervention in early phases of psychosis, a finding with relevance for resource-constrained settings where choice of medications may be limited. Resulting from this body of works are ongoing efforts at integrating guidelines with best fit to the African context into the existing mental healthcare systems.
