Brief Summary :A recent study showed that structured discharge planning practices, policies and procedures are lacking in the clinical settings. Hence, the need for a current educational intervention to optimize patients’ discharge process among nurses. The study is providing training for nurses in the medical wards of tertiary teaching hospitals on how they can coordinate and plan patients’ discharge right from the point of admission till post-discharge period. This study intends to improve knowledge, and practice of discharge planning among nurses and consequently reduce the preventable post-discharge adverse events (such as, frequent readmission, medication errors, difficulty in following discharge instruction) in patients.Publications
