VIII. DETAILS OF TEACHING EXPERIENCE AT UNIVERSITY LEVEL:

a)   Undergraduate Training

Since my appointment, I have been involved in substantial teaching duties to clinical students in the Department of Obstetrics and Gynaecology at undergraduate level, and at the postgraduate level for the various masters programs run in conjunction with the public health faculty. The duties included formal lectures as allocated within the departmental lecture program. I also carried out tutorials, bedside teaching which, involved demonstration of clinical signs and symptoms of various diseases to clinical students on the wards, clinics, accident and emergency unit and the labour ward. I have in the past 6 years served as internal examiner to the department of obstetrics and gynaecology during end of posting tests and the part III final MBBS examinations which I have been coordinating for about 5 years now.

I have supervised 8 Medical student group projects.

I have also been external examiner to the Department of Obstetrics and Gynaecology, University of Benin.

I give lectures and supervise seminars for the Postgraduate students who are registered for the Postgraduate Diploma in Population and Reproductive Health, Master of Science in Human Reproductive Biology (MSc) and Master of Health Science (MHS) in Population and Reproductive Health  programmes  of the Department of Obstetrics and Gynaecology.

I have supervised   postgraduate dissertations.

I have been a resource person at the Basic Sciences Revision Courses and Update/Revision Courses for Part 1 and 2 of the Faculty of Obstetrics and Gynaecology of both the West African College of Surgeons and National Postgraduate Medical College of Nigeria since 2004.

I have since 2006 been a resource person at the yearly Intensive course in Reproductive and Family Health organised by the Department of  Community Health,University College Hospital,Ibadan.

I have been an Examiner to the Fellowship Exaninations  of both the West African College of Surgeons and National Postgraduate Medical College of Nigeria since 2006.

I have been the Departmental Coordinator for the Part 1 and Part 2 examinations  of  the West  African College of   Surgeons examinations in Obstetrics  and  Gynaecology since October 2007.We host the Examinations in Ibadan April and October every year.

ACADEMIC PUBLICATIONS / ACTIVITIES:

My area of Research interest is Fetomaternal Medicine and have published works on Medical condition in pregnancy.

I have authored / co – authored 3 dissertations, 52 journal articles and co-authored  a book. I have made presentations at several local, national and international conferences and workshops.I have 60 published conference abstracts. Some of our research findings have been used by the World Health Organization and other organizations to update medical knowledge and practice.

My teaching, tutoring and examination experience involves students of Medicine, Public Health, Nursing and Midwifery both at the undergraduate and postgraduate levels.

I have supervised  postgraduate dissertations and supervised 8 Medical student group projects.

PROFESSIONAL DUTIES AS CONSULTANT OBSTERICIAN AND GYNAECOLOGIST:

Since 2006, I have been a Consultant Obstetrician and Gynaecologist in the department of Obstetrics and Gynaecology, University College Hospital, Ibadan. In that capacity, my duties include outpatient and inpatient care. I also perform Gynaecological and Obstetrics surgical procedures. I am also involved in teaching Resident Doctors in training for professional postgraduate diploma, i.e. FWACS and FMCOG programs. I have supervised dissertations of some of them at the part II Fellowship examination level leading to the award of FWACS and FMCOG.

SUMMARY OF PUBLICATIONS 

1. MATERNAL – FETAL MEDICINE

This is my area of interest and of sub-specialization. Majority of my research work is in this area.

PRECONCEPTION CARE

Publication - Numbers 50,51

These papers look at some issues prior to conception that may lead to hypertension in pregnancy.

We found that increased length of sexual cohabitation prior to conception reduced the risk of gestational hypertension.Another study on influence of previous abortions and new paternity as a risk of hypertention in nulliparous  parturients supported the immunological theory of the aetiology of hypertension in pregnancy.

ANTEPARTUM

These group of papers looks at conditions that impact on antenatal care.

We found out that domestic violence is common in pregnancy and that the husband is the most frequent assailant. Domestic violence was also studied with the experiences of obstetrician and gynaecologist. The causes were elucidated and the role of the obstetrician was evaluated. The paper concluded that the obstetrician had a medical and ethical obligation to recognize and intervene on behalf of the abused patient.This paper was quoted by Amnesty International.

The review article on anaemia in pregnancy looks at the significant impact on anaemia on the health of the foetus and mother.

Vulva carcinoma is very rare in pregnancy.This report is one of the very few that have been published that follows its progression from genital warts to vulva carcinoma.

The vital role of education is important in prevention of the HIV and reduction of the chances of high-risk behavior.

Chloroquine until recently was the safest and most utilized drug in the treatment of malaria in pregnancy.Its use was hampered by the pruritus it produces. This study addresses the prevalence of this pruritus in pregnancy and shows that its usefulness was not hampered by it as majority of women use it despite this side effect.

Malaria was also studied in the antepartum period in a secondary health centre with surprising low prevalence compared to previous reports.

The commonest reasons for having coitus in pregnancy were marital harmony and facilitation of delivery.

In our two studies on urinary infection in pregnancy, the prevalence of asymptomatic bacteriuria was high among antenatal patients with a common incidence of positive culture in urinary tract infection in our patients. 

ANTEPARTUM/INTRAPARTUM

These  group of publications address maternal conditions and issues that occur in both the antepartum and intrapartum areas of study.

Obesity which has long been recognized in the western world as having an impact on obstetric performance was studied in this environment. Its prevalence was much lower than in other parts of the world but its complications were similar.

Maternal hematocrit to detect anaemia in pregnancy and its trend and impact on pregnancy outcome was studied prospectively. The paper was recognized by journal choice as one of the best for 2003 (letter of recognition by the editor of West African Journal of Medicine is attached to the paper).

Hypertension in pregnancy was studied in an attempt to find predictive markers. Microalbuminuria was found as a more promising marker than serum albumin, creatinine or uric acid. These papers have been included in systematic reviews.

The study on pregnancy outcome in diabetics showed that good glyceamic control was achieved in majority of patients and that good control reduced fetal morbidity and mortality. It also resulted in better Apgar scores at birth.

The paper on term quadruplet pregnancy demonstrated that quadruplet pregnancies can be delivered close to term with the advantage of improvement of perinatal outcome.

The study on the rare condition of abdominal pregnancies reviewed the modes of presentation, diagnosis and modalities of treatment.

The review article on antepartum haemorrhage looked at this important complication in pregnancy. It reviewed the causes, clinical features, diagnosis, management and complications of this condition.

The study on male participation in pregnancy and delivery aimed to assess the level of participation of Nigerian men in pregnancy and chid birth and the attitudes of the women and likely targets for improved care delivery. The study showed a high level of awareness of women to male participation.

Our study on plasma c-reactive protein and selected nutritional indices in elective caesarean section found that adequate nutrition was one of the good predictive indicators of good outcome after caesarean delivery.

ANTEPARTUM / INTRATUM / POSTPARTUM

Hypertension is a common condition in pregnancy, the prevalence in pregnancy, delivery and postpartum period was evaluated using a cohort methodology. The incidence of hypertension was significantly higher post partum than at booking. Previous personal and family history of hypertension was predictive of hypertension in pregnancy.

In another study women who developed pre-eclampsia in the course of pregnancy had increased oxidative stress.

The review article on hypertension in pregnancy addresses current knowledge on the condition including our contributions to current knowledge.

INTRAPARTUM

These group of papers addreses issues and conditions in the intrapartum period.

We looked at  pregnant women and their experiences at caesarean section. We found that most women were involved in the decision to accept caesarean section.

It is possible to reduce perinatal mortality in twins by reducing the incidence of birth asphyxia and low birth weight.

Obstetric analgesia formed a major part of papers in this section. Attitude to it in antenatal patients, determinants of pain scores in labour and its implication for analgesia, also the growing influence of westernization in our environment informed a study on its influence on pain perception in labour. The influence of parturients’ ethnicity on perception of pain in labour was also studied with findings showing lower pain perception for parturients delivery in centres where they share ethnicity with their caregivers.

Uterine rupture a cause of maternal and perinatal mortality in this environment was found to be on the increase when compared to the decade preceding the one in which the study was conducted. Instrumental vaginal deliveries including destructive operations were studied and the art was found to be on the decline and current roles in obstetric practice were highlighted.

Randomized clinical trial was performed and showed the superior efficacy of Misopristol over the prior standard practice of foley catheter insertion in pre-induction cervical ripening.

POSTPARUM

Determinants of rates of uterine involution post partum were studied using ultrasound. It was found that parity was the most important correlate of the rate of involution.

Social support during breast feeding was studied and beneficial cultural practices that encouraged adequate breastfeeding were identified.

Low parity, poor education, low income, no or poor antenatal care were some factors responsible for retained placenta.

2.ADOLESCENT SEXUAL AND REPRODUCTIVE HEALTH

Book 1

This training manual on Sexual and Reproductive Health and Rights and HIV prevention for Medical Students in Nigeria was developed as a component of a package targeting improved Adolescent Sexual and Reproductive Health(ARSH) / HIV / AIDS prevention information and services to adolescents and young persons.

Despite the high awareness of female genital cutting only a few people knew about its health consequences.

Many University students were sexually active and had little knowledge on contraception. Female undergraduates had a poor awareness of emergency contraception and there was poor usage,while a significant unmet needs remains.

The study on immunization showed that almost 80% of children were fully immunized. We concluded the role of antenatal clinics as a source of awareness should be further strengthened by training more health workers.

Our case control study on adolescent pregnancies found that psychological, nutritional and social work services should be an integral part of obstetrical care especially in a low resource country like Nigeria.

3.GENERAL GYNAECOLOGY

These group of papers addresses my interest in general Gynaecology.

The first 2 papers found out that most women were aware of litigations in Obstetrics and Gynaecology practice and are willing to sue in case in the event of medical negligence.

We reported a rare condition of Ogilvie’s syndrome which requires a high index of suspicion to make the diagnosis. Most patients can be treated non-surgically.

We studied Pelvic abscess extensively .We looked at  the socio-demographic risk factors, the under-utilization of colpotomy and microbiological content to assist clinicians in commencing antibiotics before obtaining microbiological result.

The pattern of abnormal semen analysis result was studied. Abnormal semen quality remained a significant contribution to overall infertility in our environment with asthenozoospermia being the most common seminal quality abnormality.

Uterine fibroids , a very common condition in this environment was also studied in this section. The previous thinking that myomectomy was associated by more blood loss compared to Total Abdominal Hysterectomy was not confirmed by this study. The study also showed a mathematical relationship between uterine size and blood loss in myomectomy which was not present with Hysterectomy.

4.GYNAECOLOGICAL ONCOLOGY

Vulva carcinoma is very rare in pregnancy. This report is one of the very few that have been published that follows its progression from genital warts to vulva carcinoma.

As regard ovarian cancer we found that prevention appears to hold the most promise in the fight against mortality from the disease in our environment. Total ovulating periods was not found to contribute ovarian carcinogenesis.
