Biography:

Prof. Adegboyega AKERE, MD (Ukraine), FWACP, DTMH (Lond), Cert Gastro & GI Endoscopy, FRSTMH, MACG

Adegboyega Akere is a Professor of Medicine in the Department of Medicine, College of Medicine, University of Ibadan and a Consultant Physician/Gastroenterologist to the University College Hospital, Ibadan. He had his basic medical training at the Vinnitsa State Medical University, Vinnitsa, Ukraine and postgraduate medical training in internal medicine and gastroenterology at the University College Hospital, Ibadan.  He also had training in tropical medicine at the London School of Hygiene and Tropical Medicine, UK, as well as advanced training in gastroenterology and gastrointestinal endoscopy both in Tel-Aviv, Israel and Bangkok, Thailand.

His research interests are in the area of Gastrointestinal Bleeding, Hepatitis B & C virus infections, Liver Cancer, Gastrointestinal Endoscopy. He was the Local Principal Investigator for the international multicentre Haemorrhage Alleviation with Tranexamic Acid-Intestinal Tract (Halt-it trial). He was also the local PI for the nation-wide study on GERD (Gastroesophageal Reflux Disease in Nigeria: A Population-Based Study)

Prof. Akere is a resource person and an Examiner with the West African College of Physicians. He was the Postgraduate Coordinator in the Department of Medicine, College of Medicine, University of Ibadan, Ibadan. He is a peer reviewer of some local and international medical journals.

Prof. Akere has published a book titled ‘An Atlas of Upper Gastrointestinal Endoscopy’. He was also one of the authors of a textbook of medicine titled ‘Essential Textbook of Medicine’. He has published several articles in reputable journals and has participated in many local and international conferences. He is a member of the Society for Gastroenterology and Hepatology in Nigeria (SOGHIN), American College of Gastroenterology (ACG) and a Fellow of the Royal Society of Tropical Medicine and Hygiene.

Ongoing Research

1. Title: When are colonic polyps more detectable, during colonoscope insertion or withdrawal?
Brief Summary: Colonic polyps are precursors for colorectal cancer and so, polyp detection rate is one of the quality measures of an effective colonoscopy. All polyps must be removed during colonoscopy because of the risk of cancer. The objective of this study was to determine when polyps are more visible. Data collection is ongoing.
Potential: If it is established that polyps are more visible during withdrawal, then it would be advised that more time should be spent during withdrawal phase of colonoscopy

2. Title: Is there any correlation between the presence of alarm symptoms in patients and clinically significant upper gastrointestinal endoscopy diagnosis?
Brief Summary: Alarm symptoms, like weight loss, recurrent vomiting, early satiety, haematemesis, melaena and others in patients with dyspepsia are believed to connote serious underlying disease, especially malignancy. So, this study aimed to evaluate the correlation between the presence of these symptoms and clinically significant endoscopic diagnosis. Data collection is still ongoing.
Potential: If this correlation is established, then it will make endoscopy to be mandatory in all the patients presenting with any of these symptoms.

4. Title: Evaluation of the frequency and type of proximal lesions in patients with distal colonic tumours.
Brief Summary: Sometimes, patients with distal (rectum, sigmoid) colonic tumours may have proximal lesions which may be overlooked if the colonoscopy is not complete. This may constitute a serious error as far as the management of such patients is concerned. The aim of this study is to evaluate how common are these proximal lesions and their nature. Presently, data collection is ongoing.
Potential: The finding of this study will highlight the importance of complete colonoscopy in patients with distal tumours.

5. Title: Analysis of the factors responsible for delayed endoscopy in patients with upper gastrointestinal bleeding at the University College Hospital, Ibadan
Brief Summary: Most guidelines recommend upper gastrointestinal endoscopy for patients with symptoms of UGI bleeding to be carried out with 24 hours of presentation for optimal management. However, in our setting this is not possible in most of our patients. This study therefore aim to identify some of these factors that prevent this category of patients from having early endoscopy done. Data collection is in progress.
Potential: The outcome of this study will highlight the factors responsible for delay in performing early endoscopy in such patients and possibly proffer solutions.

6. Title: Pattern of appearance of appendiceal orifice during colonoscopy at a tertiary health facility in southwest Nigeria.
Brief Summary: Appendiceal orifice is one of the landmarks to identify caecal intubation during colonoscopy. However, the appearance of the orifice varies from one patient to the other and may even cause confusion for some endoscopists, who may mistake some appearances of the appendiceal orifice as an abnormality. So, the aim of this study is to describe the common normal appearances of the appendiceal orifice in our setting.
Potential: The knowledge of various normal appearances of the appendiceal orifice during colonoscopy will prevent misdiagnosis with attendant mistreatment.

7. Title: Correlation between endoscopic and histologic Barrett’s oesophagus.
Brief Summary: Barrett’s oesophagus is one of the complications of GERD and is a premalignant condition for oesophageal cancer and so, proper diagnosis is of utmost importance so as to prevent the development of oesophageal cancer. Although, histology is necessary for confirmation of this diagnosis, but sometimes Barrett’s seen at endoscopy may not be confirmed histologically. So, this study aimed to evaluate the frequency of such disparity.
Data collection is about to start.
Potential: The outcome of this study may prove if Barrett’s oesophagus is being over-diagnosed at endoscopy.

8. Title: Evaluation of the usefulness of hepatitis B virus genotypes and the risk of hepatocellular carcinoma.
Brief Summary: HBV infection is the major aetiology for HCC in our setting. There are six major genotypes of this virus and some of the genotypes have been described as aggressive. Specifically, genotypes C and D have been shown to run an aggressive course and are associated with early development of HCC. So, the aim of this study is to describe the genotype(s) most prevalent in patients with HCC. Patients recruitment and data collection are ongoing.
Potential: If it is established that, the aggressive genotype(s) is (are) found in the patients with HCC, then determination of HBV genotype will be suggested as one of the mandatory tests for patients with HBV infection. So, regular monitoring and probably treatment will be suggested for patients with HBV infection found to have any of these genotypes. This is to prevent HCC.

9. Title: Evaluation of the safety and diagnostic utility of percutaneous liver biopsy in patients with liver diseases at the University College Hospital, Ibadan
Brief Summary: Liver biopsy is one of the invasive procedures commonly used in hepatology for histological diagnosis of some liver diseases. However, many gastroenterologists are shying away from it due to its invasiveness. The aim of this study is to describe the diagnostic efficacy and correlation with clinical diagnosis, as well as its safety. Patients recruitment and data collection are ongoing.
Potential: The result of this study will prove the safety of this procedure, as well as correlate clinical diagnosis with histological diagnosis at our setting.
